I managed to get a red cross in an empty slot. You can see the chest on aquilo in the image below.
I don't know how exactly I ended up with this but I was in remote view and marked stacks of items for deconstruction and cancelled it again while robots interacted with the chest. I made the save right after I confirmed that other players could see the cross in the empty slot as well (1-2 minutes after its occurance at most).

I have encountered this a few times in a Local game (ie, no Multiplayer lag involved), not able to reproduce reliably. It seems like it happens when the "Deconstruction Order" for the items is canceled in the same Tick as multiple Robots are interacting with the Slot / the Stack count goes to 0.
I have never been able to reproduce in Editor Mode (Paused), or with "Tick N times". I suspect that it is something about the order in which changes are applied during a Tick?

Stumbled upon another way to arrive to the same red cross on empty space situation:
1. Have a single storage chest in logistics network or a storage chest not in range of any logistics network
2. Put a blueprint item inside (not an empty one, but one with some entities on it)
3. Via remote view mark blueprint for removal with left click
4. Via remote view or in person right click the blueprint in chest and in blueprint dialog press "Destroy blueprint" button
5. You will end up with a cross on an empty slot
Other things I've noticed:
- If player will interact with storage chest in some way then the red cross will disappear.
- If inserters will interact with storage chest or bots will put items inside the chest then the red cross will stay and will be ignored as long as bots don't have any place to move items away.
- If bots will find some place to put "unwanted" item (you will add another storage chest to the network for example) then
-- if the cross is still on the empty space - a single bot will go through the removal motions without carrying anything
-- if the cross is now on a stack of items (because bots or inserters filled that slot) - bot will remove a single item from this stack
