In some edge cases, inserters with specific offsets set (thanks to bob's inserters) do not seem to be able to balance materials when you would expect them to be able to. I'm reporting this as an engine bug as this is a behaviour in the underlying inserter logic and Bob's mods just change inserter pick-up and drop-off positions.
The bug is demonstrated in the attached save (log also attached). The save file has a lot of mods as it's from my Seablock game; I've not attempted to reduce the mods because this bug is very sensitive to conditions.
The bug is very simple to see when you load the save: the crushed stone being removed from the warehouse should be balanced, with output on both sides of the belt. It is not; instead, only one side of the belt is being filled at all. You can see this behaviour working correctly on the adjacent belt.
This situation is not entirely stable, and correct behaviour can be restored by various things, such as changing the rate of stone production, adding some extra crushed stone to the warehouse, or deleting and recreating the inserter that is placing the stone (oddly enough, deleting and recreating the other inserter does nothing). It also seems that the situation will reassert itself after some time in some cases, but not always. Regardless, it is very sensitive to the exact rate of production.

Thanks for the report however this has nothing to do with custom inserter pickup/drop locations and is not a bug. 69646 inserters don't take turns.
