Place it on your toolbelt and click when you are low health

You need to then click yourself to actually eat the fish, but yes, that's how it works.

You don't need to click "on" anything specific, you just have to have the fish on your cursor and left-click.

Pretty sure when I've tested it in the past, I actually had to click on my character, but I may be mis-remembering.

you can also throw fishies at other people, it will not feed them, tho'
