This isn't a bug. The fast serter is just picking up the plate before it goes through the splitter.

Yeah try it again with 2 or more inserters placing onto the belt, so the 2nd one gets a chance to grab one as well.
Also, you don't need a splitter for this sort of setup, just turn a regular belt 90 degrees and go one more space.

Not exactly.
