My mod list got into a strange state. I had 1.1.1 of Calculator UI installed, which wasn't Space Age compatible, so I went to the Update tab and downloaded all available updates. This seemed like it downloaded 2.0.1 of Calculator UI, which would be Space Age compatible. But when Factorio restarted, the Manage tab showed 1.1.1 was installed, the Explore tab showed 2.0.1 was installed, and the Update tab showed no updates available. Calculator UI was not usable in-game.
I resolved the issue by clicking the red trash can icon next to Calculator UI on the Manage tab to uninstall it. When Factorio restarted, it showed on the Manage tab that 2.0.1 was installed without it having to be downloaded again, and after that the mod was usable.

thanks for the report however I'm not able to reproduce any such issue. There is 1 scenario where the game won't delete old versions when updating and that's if you already have multiple versions installed due to joining some server(s) that require specific versions.
The only other scenario that I can think of where it would fail to remove an old version is if on-exit the request to delete the file is blocked by the operating system or some other program on the computer.
