What did you do?
Starting up oil in Space Age, I built my refineries with pipes for the eventual output of light and heavy oil. When I researched advanced oil processing, I changed the recipes, enabling the additional fluid outputs.
What happened?
Two of the three outputs connected properly, but the straight pipe did not convert to a T-pipe and was not connected to the refinery output.
Rebuilding the pipe solved the issue for that refinery. Additionally, that pipe was not connected to the adjacent underground pipe, which was filled with heavy oil, even as I worked up the row to fix the connection issues.
What did you expect to happen instead?
Changing the refinery recipe should connect to pipes in place to handle outputs. Fluid should propagate through connected pipes / underground pipes
Does it happen always, once, or sometimes?
It happened to all of the refineries in that row. I have not attempted to reproduce the issue.

Thanks for the report however i am not seeing any issues here. You have pipe-to-ground entity near the heavy oil output. That is not a pipe.

The underground pipes are to the sides of the heavy oil output, with a single regular pipe (highlighted in the first image) between them to connect to the output. It's the same setup as the light and gas outputs, just adjacent to the refinery rather than with a spacing pipe.
As mentioned, deconstructing and reconstructing the pipe connects it properly, which is what made me think this was a bug
[edit] Unless, as you say, it's an underground pipe and I can blame my multiplayer partner. That is a possibility

This is a pipe-to-ground. It can be recognized by the sprite and by the visualization with the dot because the fluid segment goes into the ground (bulb). Also you may notice it does not connect directly to the entity below (other pipe-to-ground) as well as to the entity on the left (refinery)
