I was building a factory's oil production up, and I ran across this moment.
I ran out of rail while planning out my path to an oil field, so I finished the rest with ghosts. I figured I would place down signals to prepare for my return trip with more rail, but the chain signal circled in the image was going to attach to the piece of rail to it's west, but after placing all rails that chain signal location attaches it to the north-east (which is more desirable anyways).
Keep in mind this is a pretty modded run of the game, but nothing should be changing rails, rail signals, and chain signals (outside of possibly Logistic Train Network but I'd find that unlikely?).
I feel like I got a secret achievement for finding a bug in this masterpiece of a game.

No achievement for you though, the rotation defines what the signal connects to, not the location.

You can hit the rotation key to change which tracks the signal connects to.
