I pasted a blueprint of a ship, that blueprint had efficiency modules in the chem plants and furnaces. Modules were sent up to the ship but wouldn't actually go into the machine(s). I tried clearing the blueprinted modules and re-adding them manually (not via BP), and it still didn't work. I tried deleting the entire furnace and recreating it and manually adding the modules, and it still didn't work.
Eventually, I tried dropping all of the modules from the ship back to the surface, then sending them back up and they were immediately inserted.

This Save appears to be from after you have un-blocked the Upgrade, which makes determining what went on in the past difficult. The ship "Vulcanus" has a pending (unfulfilled) request for an Assembling Machine 3; this was probably stuck "before" the pending upgrades in the Chemical Plants.

So that save file should've had the bug in it. When I load that save, the "Vulcanus" ship still won't allow the modules into the machines. I loaded the game and created a new save where it is/was still present and attached that here.
