The holmium solution side of the EM plant was likely already contaminated by heavy oil. You can fix it by clicking the pipe and flush the heavy oil. Then even if you remove the pipe and place it back down again, no more heavy oil? It would be a case of fluid residing in the fluidbox of the EM plant, but you can't see what's in it.

This looks vaguely related to Raiguard's "fluid stashing" that was supposed to make it easier to recover from pipe mixing scenarios without losing materials.
IIRC, when two fluids end up in the same pipe segment, the smaller of the two gets "stashed" and when the first fluid drains completely the stashed one is reinserted in the pipes. I'm not really sure how it's supposed to work and it is not made clear to users what is going on. Raiguard said he might remove this feature in the future and simply delete accidentally-mixed fluids.

IMHO Wube should first at least try displaying this information, before/if they decide to scrap this idea.

I found at least one way to reproduce this. I encountered a bug today and came here to report it, but this thread seems similar enough that I'm not sure if I should create a separate thread or if we can move this one out of "not a bug".
When you have an EM plant set up with a recipe that uses 2 different fluids (exhibit A), and you switch the recipe to one that only uses 1 fluid and lets it pass through the factory, it will flush one of the fluid systems with the other (exhibit B)... even if the new fluid required by the factory does not match the fluid that was flushed through.
