Hard to describe in words so I'll use pictures instead. On the left I have a bunch of stuff facing east, and I'm about to hover a horizontally flipped version (shown on the right) over it.
I expect all of that to block, so I expect it all to be red. Except the underground belt appears in blue and facing east, despite it facing west in the blueprint.
So, given that it's blue, you might expect pasting an underground belt over one facing the opposite direction will switch its direction, but it doesn't, it simply blocks. So this is purely a graphical issue.
It always seems to happen regardless of orientation or type, though I've not done extensive testing.
I found one bug report that might be related, but it's not the same issue:
viewtopic.php?p=623996 [2.10] Holding an underground belt over an existing underground exit, the arrows point the wrong way.
In case it matters, I'm playing on Windows 11 with an RTX 3060 (notebook), driver version 560.70. Hope this was helpful :D

Oh hey, it happens in 1.1.110 as well, I'm surprised I didn't notice it earlier.
