Loaded the game and started placing blue belts when the screen flickered to black and a popup window appeared:
Swap buffers failed.

Hello, please post your log.
Press Win + R, type in %APPDATA%\Factorio and click OK. File Explorer should open and you should see file called factorio-current.log.

When started with the "--force-opengl" option, the game is loaded, but with a "black screen" (no image). In this case, there are a lot of "ERROR 'INVALID_OPERATION' (1282) in glDrawElements" messages in the log.

Probably due to the small amount of video memory in my laptop or an old and unsupported driver (AMD has not updated the driver for the Radeon HD 4200 since 2013), hardware creation and compression of mipmaps causes difficulties. To start the game, I do this:
  • first use the option "--gfx-safe-mode" to start the game and create a configuration file
  • then change the "gpu-accelerated-mipmap-compression" option to "false"
After that, the game starts at least.

I have the same with
* Current 17.25
* On a 1808Ti with 11gb memory

So it definitely is NOT a low on memory scenario.

Today happened to me too, right after i tried to choose save slot. Factorio 0.17.31, no mods. First crash ever (~480 hours).

Spec:
Win10 64bit
Processor: Intel(R) Core(TM) i5-8600K CPU @ 3.60GHz (6 CPUs), ~3.6GHz
Memory: 16384MB RAM
Card name: NVIDIA GeForce GTX 1060 6GB
Drivers: 419.35

After hard reset, game works perfect.

This has happened to me also.
OS Name Microsoft Windows 10 Pro
Version 10.0.17763 Build 17763
i5-4690k
NVIDIA 970, Driver v 388.13
8 gb ram
Mods: Rampage and Longer Range Turrets (Turning these off while debugging now)

Factorio: 0.17.34


Interestingly, this started happening after a windows update. However, there are also multiple factorio updates that have happened also. I'm going to turn off all mods and wait for it to happen again. I'll start making 1 change at a time and hopefully I can pinpoint what is causing this.

This happened while rotating/placing blue belts. Was combining several belts worth of output into a compressed blue belt. This has happened 2-3 times now. Cant remember what version it started though.

Windows also reported these errors at the same time as the application crashing.

Edit: 5-6-2019 -> Happened again with all mods off, took a week though. Updating graphics drivers to latest (430.89) since I'm still rocking 388.13 and will wait and see if it happens again.

Edit: 5-7-2019 -> Happened again w/ my drivers updated to the latest. Going to try --force-opengl and see if that helps

Thanks for all the reports.

By now, I am 99% sure it is not caused by a bug in the game and it is rather just another error that just happens on some computers due to some instabilities.
It could unstable graphics driver, other software messing up GPU state, or HW being unstable for some reason - overclocking, overheating, power issues, manufacturing defect, you name it. Updating graphics driver while the game is running would cause this error also. If you try to lookup the error in context of other games, you find the wierdest advices that people say helped them to fix the problem - for example disabling all HDMI Audio devices.

Anyway, if the error occurs, you should try to just reboot your PC first; If that doesn't help, try to update your graphics driver, and if that still doesn't fix it, switch try to switch to OpenGL renderer.

I also gave up on trying to recover from the error for time being, so when the error occurs, a better error message will be shown with an option to switch to OpenGL as a workaround.
