Great game. Love it.
Had been running a save from 0.15 with about 200 hours headless on linux with zero issues.
Upgraded to 0.16.18 and played several hours with just one player in the multiplayer with no problems.
Then:
1) Second player joins and builds new train outposts surrounded by laser turrets for about 30 minutes
2) We send our artillery train and it fires many shells at nearby biter nests
3) Hundreds of bugs run at the laser turrets and get mowed down
4) Beautiful
5) First desync ever
Creative Mode mod was installed on first players machine but disabled the entire time and never used on this save.
After rejoining (and after deleting all mods just in case):
1) Moved train away from forward firing positions
2) It loops back around
3) Fires
4) Many bugs die
5) Desync 2
After rejoining again:
1) Moved train away from forward firing positions and disabled
2) Waited
3) Straggler bugs may be getting shot in the background
4) Desync 3
I'm not sure what the root cause is but we were unable to play for more than a few minutes before each desync after the first.

Edit2: After upgrading to 0.16.19, joining players desync almost immediately (desync report)
