Basically, for modded games achievements are not saved between game launches.
Steps to repro:
1. Ensure some mods are enabled
2. Load a save with some achievements, ensure they are unlocked
3. Restart the game
4. Load the game again, notice how at the start achievement screen shows no achievements are unlocked, and then you start unlocking them again
Vanilla achievements work as before. Also, modded achievements do not reset if you just quit to main menu and load/start another game.
I have attached a save and few mods with which I had this problem.

I tried to reproduce it, and it worked properly for me.
Loaded save with mods, got achievements, exited Factorio. Started Factorio, loaded game, and achievements were still there.
Could you show me the log? (IE are you using steam?)

Yes I am using steam version. I've redone the steps (with same incorrect result) and attaching logs from them.

Just an update - I've checked the behavior for standalone (non-steam) version, and it worked correctly for me as well. I saw both achevements.dat and achievements-modded.dat in data directory. However on my steam version these files don't exist anymore. I believe it because of this change:
Can it be that modded game achievements are not being saved in cloud at all, only vanilla ones? I don't really care about achievements per se, but having 10s of them loudly pop up every time I start a game can be... annoying.* The game uses only the cloud version of player-data and achievements on steam to avoid problems with resets.

I also got the "It stinks and they dont like it" achievement now multiple times on my new map whenever a first attack after game restart happens.

Just a note, i don't have achievements.dat file at all, and it's not being regenerated into \AppData\Roaming\Factorio\. I am getting achievements everytime i start my modded game.

This bug appears from a Full quit and restart of the game. Basically every time you launch and play factorio.
If you quit and load a game without shutting down the client the modded achievements are still kept or saved.

Thanks for the report, it is fixed for 0.16.8 now.
