I'm not sure if this is related to the weird path finding issue I've seen bug reports for, or if it's related to this bug: viewtopic.php?f=29&t=44108
I noticed that some trains that had crashed into each other. I'm not sure if they crashed because I removed and placed new tracks far away from this spot, or if it was the other path-finding issue, because the time between me placing tracks and noticing it was probably a solid half hour, or more.
Here's the save I made when I spotted the collision, on the multiplayer server:
