I fear this one could end up in 1/0 magic but the behaviour is repeatable, although I haven't exactly found out what causes it (yet). My test setup has been running since version 15.18 or so and I am now on 15.20.

Problem: My kovarex setup was built on a looping belt where the centrifuges output after input. Thus, a full 40 has to always loop around to ensure the first one has enough to run. So I set up a counter and when there is more than 40 in the belt section monitored, the inserter was allowed to run and stack bonus size was set to 1. To achieve this, I count the pulse of 235 at the input and multiply the pulse of 235 at the output with -1. An arithmic set at U235 * 1 output U235 is used as memory cell from these inputs. My counter wasn't reliable however. Sometimes it would end up over 40 with no U235 on the belt, sometimes it wouldn't go up. However, it was very hard to be certain that there wasn't an error in my setup and it was uncontrolled. Replication: I decided to try to isolate the problem. Specifically, I wanted to test a number of things:
  • pulses with a backed up belt
  • frequent pulses at regular intervals
  • pulses in batch
  • delays in the counted section
The screenshot is the bottom part of a 39 piece setup. (9 setups are unused). From top to bottom, every setup was set with a different stack bonus. The top ones inserted on the belt with stack size 1, the bottom ones with stack size 12 and everything in between. All inserters were started simultaneously by enabling power to the entire column at once. All input boxes had exactly 12 of U235 (and U238 in the middle setup). From right to left I tested: base case with simple U235 running around, mixing 238 onto the same belt and an inserter to create delays. In the top left (not shown on screenshot) I have 3 setups that allow the belt inside the counted section to run for 1, 5 or 10 ticks per second.

The lamps indicate the number of U235 in the counted section (In between the right hand and left hand read belt). As there is 12 U235 in total on the belt, none should have more than 12 lamps activated at any time. When there is no U235 in that section, all lamps should be out. I have continued playing while occasionally monitoring the setup. I'd say that it's now been running for over 12 hours of game time. I have been unable to spot WHEN the error occured, but I have definite proof that the error exists.

The following setups now all show incorrect values (the ones in the screenshot are circled with red):
  • Base U235 loops in stack size 1, 2, 9 (negative counter), 10 (negative) and 12
  • Mixed U235 & 238 in stack size 3 and up (all 10 of them!) all show negative values when there is no U235 in the section
  • Inserter setups stack size 3, 4, 10 and 12
  • Belt run for 10 ticks per second (1 lamp always on)
I believe I have simplified the setup as much as possible but I'd be happy to run more tests if you can discern a pattern that I'm missing.
Could this be a result of UPS < 60? Most of the time I'm at or close to 60 but when I go through trees with my train it does drop to 30 if I'm not fully zoomed in.

UPS won't affect the counter, most likely.

Are the belts set to pulse or hold their contents?

I'm very confused about the explanations.

I'm very confused about this list. You enumerate some things but only circled 5 setups in the screenshot.

I took the setup in the bottom right and ran it for 18 hours and there was no problem.

Ok, I figured out the problem.
Your base would occasionally run out or have electricity disconnected. Belts continued to run but inserters and combinators not, leading to these inconsistencies.
