I've seen some people talking about pollution and medicine so I thought I should suggest something. Pollution would have some negative affects on both the players health and local wildlife so you would have to keep the environment clean and deal with illnesses via medicine which would be made with herbs found in the environment. If there's too much pollution the herbs wouldn't be effective and may even worsen the situation so the player could craft a greenhouse to grow plants in convenience without having to worry about the environment. :D

looking into old threads...

I thinks this belongs to an epic, which can be called "producing food and medicine", "character needs" or so... which I'm a big fan of, especially, when it would be implemented with the richness of details kind of game-art, we already know.
