Smart Inserter - "Pickup All BUT"

Is there a way to setup a smart inserter to say "pick up everything "BUT" this 1 item" ?

That is a workaround that can work in some situations, but could quickly become too cumbersome.
I see no reason why there shouldn't be an option for an invert filter. Who knows, maybe it's even planned with the additions to the circuit system in 0.13...

Basically you want a Blacklist and Whitelist option for the smart inserters.

Another workaround is, place a smart inserter after it, picking up the non-desired item and feed it back to the main belt.

This is a much larger version of what you're looking for. It's not an inserter, but it's a filter solution. Using the belt layouts in here, most of the people filter out 1 item and keep that. You seem to want the opposite.
