Biters overran my base as I was finishing the last research. After the winning the tutorial dialogue box I was not given any of the post death options, and just floated around without a body.
The order was:
Logistics science pack Research completed/Biters killed me (I'm not sure which happened first, they both occurred pretty much at the same time).
You finished the tutorial dialogue
I selected to continue the game
The two triggers happened very close together so I would not be surprised if it was just some form of race condition that caused one trigger to not take effect.
