When placing rail signals along a rail track using shift-click to clear out trees in the way, the signal is sometimes not connected to the rail, and shows the rolling light pattern (red-yellow-green-etc). Removing the signal and placing it again works fine. Shift-click when there is no tree in the way always work.
The expected behavior is to remove the tree and then place the signal and attach it correctly to the rail.

Yes, a save will be needed. I tried to replicate it from your description and did not get it.

Placing a signal from a blueprint never works for me, no matter the situation; they never connect to the rail. Placing in the same spot manually always works.

Doesn't work regardless of direction of signal or rail, have tried every combination of each.

Works flawless for me, make a new report if you want further diagnosis, as it has nothing to do with this one.
