It looks like some combination of having six connected nuclear reactors, with only three of them powered up (I haven't needed them), and trying to run all the heat exchangers from a single heat pipe. I removed two of the unused reactors, turned on a fourth, and ran separate lines to each row of heat ...

Nuclear Power Issue
I have three columns of turbines hooked up to my reactors. Previously, all three columns would spin up and provide power. Since 0.15.19, only the first column will spin up. I haven't changed anything. The heat exchangers appear to all be getting heat and water, but the turbines aren't getting steam.

Was able to replicate: it does appear to happen when I try to throw a poison capsule, which someone else has already reported.
