I'm hovering over the last tile of the underground which is one belt tile. One belt tile contains a maximum of 8 items, shouldn't the tooltip show 8 instead of 4?

Sorry, I'm not sure I understand what you are saying. In this situation:

Minor issue (IMO), but nonetheless... The item count for all underground belts (all types) seems to be 4 less than expected (always).
For example, given this belt with a length of 8 (2 normal belt tiles and 6 underground tiles):
