Decider combinator using the everything signal is used with no signal output will be true. See first picture. Does this happen right away on brand new games or does this happen after a server save? I had a similar issue but with mods a while back and increased ram resolved the issue. Sorry, you do not stamp over existing stop. This bug only happen if you create a new stop. If you stamp over an existing stop it will fix the settings. I don't know if it should be considered a bug for the LTN mod or a general Facotrio issue.
If the LTN stop gets destroyed in anyway the circuit connection is lost.

The power pole connections change depends on when they are placed.
If you want to cleanup the connections, you can use the Power Grid Comb to fix the connections and they will align correctly again.
