It happened when i was playing League of Legends- i was in champ select, but LoL window was in background and i was playing factorio.
Then, when champ select ended, and game began to load, there was some error message saying something about graphic card or drivers (i think), but i forgot to take screenshot.
This happened to me 2x,
My PC: win10xp, i5 6500, 16GB RAM, rx480 4GB
