I've found that I am able to fit a train in between two train cars of another train. This has happened to me twice now, and has both times happened while manually driving the victim train.

That's because the train *does* fit between the cars 
If you turn on the collision boxes you can see the trains have space between them.
Trying to "fix" that would add a bunch of overhead without improving the gameplay any so that's how it stays.
