Uranium Ammo Quality Bugged

A statistically practically impossible result has occurred that leads me to believe that something is bugged with quality uranium ammo production.
Setup: single assembler, normal quality uranium ammo recipe, that was originally a tier-2 assembler, upgraded in place to tier-3. Prior to upgrading the assembler, it had 2 tier-1 quality modules in it for a total of 2% quality, but it was quickly (within the first 100 ammo) upgraded to tier-3 assembler and 2 more tier-1 quality modules were placed in it. It has since crafted nearly 1000 uranium ammo. Additionally, 2 beacons with 2x tier-1 speed modules were placed within range of the assembler almost immediately after upgrading the assembler.
I have crafted a total of 1086 uranium ammo as of this writing, and only one of the early crafts was an uncommon quality ammo while the other 1085 have been normal quality. I am certain that at least 900 crafts were completed using the current setup of 4% quality, all of which have resulted in normal quality uranium ammo. The probability that 0 of the 900 crafts were of uncommon or better quality is 0.00000000000001107%, or about 1 in 9 quadrillion. It's more likely that I'd win the Powerball lottery and be attacked by a shark in the same year than that this would happen.
I'm going to replace the assembler and beacons, but I've tried to outline the process that led to this occurrence so that the bug can be identified and fixed. I'm happy to answer any questions.
