looks like your vram is fuuuuugd'
if you want to, there is this "oven baking" techique that can give you just a few more months of life out of your graphics card (my 9800gt lastet 'bout half a year after baking)

Guys I found it's somehow connected with Evolve when I chat for multiplayer. My graphic card still works very well.

Or Evolve is doing some really weird stuff.
Overclocking vram to slightly below it's hard-lock limit also looks like this. Try underclocking the mem, even when you haven't overclocked it, and see if it goes away.

FYI; i use MSI afterburner (and no, again, it doesn't require an MSI card) because it has the smoothest GUI
