I have recreated the scene of the crime. At the time I was moving east, away from the train as it actively hit me. I am pretty certain I did actually place the signal a split second before it crashed - I should know really as it froze for a few moments before returning to desktop with the dialog box.

Can you provide more detail? What was the rail layout like - did you place the chain signal before or after the train - was it arriving at a station when it happened or just driving along? I was removing a chain signal and stepped onto the line, naturally I got hit by a train which was resisted by shields.
