This is a modded issue.

Vanilla is OK.

It comes from https://mods.factorio.com/mod/MultiConcrete
The mod has only three tile rotations, but the game makes forth of them (concrete tiles are twice somehow).

No, this particular issue was picker tweaks, and the "rotate concrete into hazard" setting.
