inserter's preference is not part of settings but part of its direction/mirroring. Are you manually building normal inserters or flipped inserters? Inserter gets whatever your cursor is when building making this not a bug. It would be easier to explain with a recycler but it behaves the same with drills and inserters where cursor's flip affects flip of the entity built.

I'm manually placing the inserter over the existing one, I can change its lane preference, but originally when placing a new inserter, it suggests a default configuration. I think it would be good for them to copy it over to the new entity

I am going to move this to Not a bug as this is currently working as intended: inserter inherits flip state from cursor, not from entity being overbuilt.

I also stumbled over this and can accept that is not a bug. However, I still want to add two argument to change it: 1. When you quick replace assemblers, they preserve their rotation. 2. You could preserve inserter flipping while normal overbuilding, but force the direction of the cursor when shift+overbuilding.
