stutter (100-200ms hitch roughly) every few seconds

Hardware;
CPU - 5950x
GPU - 2080ti
RAM - 64 gigs @ 2133
Monitor - 1440p, 120hz
Settings are all set to max, tho this issue seems to be setting agnostic
When running in windowed full screen, i get lots of stutters, espeically when panning around. From what i can tell its cpu hitch of some nature, atleast, thats how it appears in task manager. very large and obvious spike.
when running at the default resolution (1080p windowed), i get no stutters. the same when running full screen. vsync doesn't seem to be working as well for some reason if thats at all helpful.
things iv done to try to fix this;
set monitor refresh rate to 60, turn on and off every setting, and set stuff like texture atlas size to the smallest, changed nvidia settings to more compatible settings, set them to default, shut down a CCX on my cpu.
the only fix i have found is to set the game to full screen, or leave it at the original boot up screen size.
