Hardcore or "Ironman" Achievement / Mode

TL;DR
The original idea was for an achievement for completing the game without dying. The follow-up was to have an Ironman mode where the save file is done after death; you have one life!

What?
The original idea was to have an achievement attainable by completing the game without a single death. An advanced version of this achievement is finishing Deathworld settings without a death.

This resulted in further thoughts of having an "ironman" mode (a combination of the same name mode in Stellaris and how a Minecraft Hardcore world is set up) where once you die, the save file cannot be continued. This can be accompanied by a unique achievement if desired as well.
Why?
This adds some serious considerations during gameplay that requires calculated risks in the very early and mid-game; obviously this becomes much easier in the late game. But certainly a few achievements for bragging rights!

There is a No Respawn mod. I was surprised that there is not a /permissions toggle for "Respawn" - maybe it is named something I don't recognize?

I would enjoy more variants of the Win-the-Game-Achievements (Launch a Rocket / Reach Solar System Edge) without dying, similar to the existing "There is no spoon" for limited hand-crafts.
