I think you mean "Transport Belt Madness" level 3...

There, fixed! Although I'm pretty sure it is actually the 5th level

It is the 5th level when playing in one go, but when starting from the menu the other 4 levels are group in 2 pairs as one level. So when so select to play level 3 you ingame get greeted with a friendly "level 5 ".
This post made me actually do the campaign so I could better judge the achievement. The moment I saw this map and how input/output chest are located I NOPE!ed out.
