I don't know why, but it should work. I change inserters and it start work for current train, but when next train come, all broke again.

you have train track placed 90° from you trains track. they are blocking the inserters. not a bug. at least i don't think it's a bug.

That would do it I might be able to "fix" the inserters so they keep looking but knowing which rail they should grab onto isn't a simple answer and the real answer is just "don't do that". So I'm going to move this to "won't fix".
