[0.17.x] High multiplayer latency

Hello.

Intro
I've been playing Factorio for a while now and every now and then i play with my friend, while me being the host. But the latency is semi-playable for my friend, sometimes to the point he refuses to keep playing. Our bases are relatively small usually, as we just stop playing after some time because of the issue. Then after few months we like to try again to see if it has resolved, it has not.

I would like to identify the problem and solve it for good. Hopefully someone here can help me. We also play other games and the latency is pretty good between us and have not had any problems to the point we have to stop playing.

Details
Me and my friend live ~10km apart, we both have 30/30 up/down connections. If i ping my friends ip, i get 10-15ms and vice versa.
Me being the host have static IP-address and can forward ports when neccessary.
My friend who joins usually gets latency of 15-30 which should be 250-500ms.
It does not matter how big the base is, the latency problem is there from the start.
My spec: Ryzen 1700 @ 3900mhz, CL14 DDR4 @ 3200mhz.
Friends spec: Ryzen 2600, CL16 DDR4 @ 2600mhz.
I have tried /c game.speed=X different values and it doesnt really seem to do much, once 0.8 seemed to do things a little better, but only briefly. Increasing it to 10 i seemed to cap out around 400 UPS on my pc. Friends latency went through the roof.
My router's DMZ is disabled, UPNP on/off doesn't change anything.

It's weird because we live so close by and normal "ping" is so low that we are basically LAN.

What to do?
Where to look?
What information to provide, so you might get ideas?
Will try to provide some logs later tonight.

What exactly happens in game that you think is caused by high latency?
Is it identical when he hosts the game/when both of you connect to a public server?
Generally factorio latency values include more than just round trip time of a ping and are not directly comparable.

Not sure, the latency is between 10-30 usually for my friend, but gets way worse if i start to unload ammo, grenades or rockets to big biter bases and they start to run around and chasing me, at the same time me still shooting at them.

He says that the high latency isn't very noticeable when we are building the factory. Laying down belts, smelters, miners, etc. The gameplay is nearly impossible for him if me or we both start clearing biters. If he is clearing biters alone, its okay, but if done together, he always rubberbands and dies eventually, because of massive lag. When i clear biters alone and he is building the base, it's still very laggy for him.

What could cause such a high latency?

Yesterday we were playing Deep Rock Galactic and the ping was 7-10ms at all times and the game ran smooth. In Factorio though, the latency is still high and multiplayer game is barely playable. Weird.
