Multiplayer-only FPS drop (60 UPS)

I'm having trouble figuring out what's causing FPS lag.
A friend with a comparable computer is able to get 60 FPS while standing in the same spot on my multiplayer server, while I'm getting ~20-40 FPS. The server is running at a constant 60 UPS.

My frame cycle is extremely high, obviously leading to dropped frames. I just don't know why. It's not even that consistent: sometimes it'll be just fine. Any ideas?
