Inserter Not Placing On Empty Belt

This seems to be happening quite often in my base, and always seems to happen off screen.

It has happened to multiple parts of my base and at random times, it's as if the belt is full and the inserter cannot place down the part it is holding.

You will see the top inserter functions 100% while the ones below do not.

Sometimes by rotating the inserter away and rotating it back, it starts to work and has a ripple effect on the other inserters, making them work again, but not all the time.

What mods do you have that affect inserters? Also, can you post the save?

Looks odd how they hover over the middle of the belt instead of the sides.
If you point your mouse at one of them and enter this command, what does it say?
(Save before if you want to keep achievements)
