See that not-fuelled rear engine? Look as train gets to the station (front engine have fuel). All the other engines get refuelled just fine, but newly placed ones like this one will not. Inserter will pick a fuel unit and just not insert it as if engine was full or had different kind of fuel.

It is expected that engine with empty fuel slot gets fuel of any kind inserted in.

Thanks for the report. The train leaves the station instantly after arriving but is blocked by the locomotive being out of power.

When a train is in automatic mode and not stopped at a station inserters aren't allowed to interact with it.

You need to set an "inactivity" condition on the locomotive of 1-2 seconds before leaving the station so it gives the inserter time to re-fuel if needed.
