The problem is solved by restarting the server, but if I leave the server and try to connect (a few hours later), I am unable to connect but instead fall directly into a desync loop.

I am likened to believe this is a mod related issue,
I will move this to desync with mods

If you can reproduce the desync without any mods,
Or if you can pinpoint a specific API call that is causing it,
Then we can take a proper look

Try removing the Axial-tilt mod and see if the desyncs go away.

Axial-tilt seems to have been the source of the issue, thank you!
