[0.15.10] Train signals showing red for no reason

Hello devs,
I've encountered a bug twice where unexpectedly rail signals are turned red permanently as soon as they are placed down. The only way to fix them are to remove and place them again.
Sadly, I cannot replicate the bug (yet).
The bug might have been caused via a mod (actually a mod I made), but the bug still exists even after reloading the save game with all mods disabled.
Bug encounter #1:

The train has "green path" but the signal right in front of the train is turned red for no reason (no circuits, no train blocking the path, and the train has a path to the next train stop).
Bug encounter #2:
I have removed the train stop the train stopped at, to make signals more visible:

The chain signal right in front of the train is permanently turned red, aswell as the normal signal a few tiles further is also permanently turned red although it is surrounded by two other signals that are green, which makes absolutely no sense.
Sadly, I was too stupid and forgot to create a savegame for bug encounter #1, but luckily I encountered it a 2nd time and the savegame for bug encounter #2 is attached in this post (one attachment with mods, one with mods deactivated).
You need to know that I placed the signals with my mod https://mods.factorio.com/mods/Burny/BurnysTSBC v0.0.8 so they were not placed (manually) by hand but instead with a blueprint. Still, I doubt the mod was the issue because sometimes using blueprints created by the mod work fine, and in these two encounters it went horribly wrong.
I hope this was a detailed enough bug report!
If I encounter the same bug once more, I will reply to this thread.
