I disagree. The belt is backed up. The coal is not moving on the belt. Why shouldn't it be able to pick it up? That's why I provided the save. Does it work for you on my save? Your save shows the end result, not how it got there. Naturally the inserter can not refuel itself if it is out of fuel alr...

They can not take from fast transport belts reliably and then run out of fuel, use normal transport belts to avoid that. NaB.

The question is, if the rotational state of the combinator is identical with the rotational state of the ghost, or in other words, if the combinators were belts, would items on ghost and not-ghost move in the same direction.

I can not reproduce this, it works flawless when I do it, from your screenshots it looks as if you placed the CC incorrectly aligned, please check that.
