So I came up with the following setup in an attempt to create an inserter that can grab only a specified number of items at a time. This only works when grabbing from belts. Grabbing out of anything with an inventory (trains, chests etc) does not work at all. A work-around is of course to put a belt ...

I completely disagree with the ops reasons for not using bots.

Low throughput is ALWAYS something that you should use bots for, because any item on a belt is useless, unless the belt is moving that item at optimal throughput.

High throughput stuff like ore/iron plates is, because they get setup ...

Thinking out loud here:
Sure you have no problems with signals?
No missing pieces of track?
Are you giving orders to the first or the second loco? (Dunno if this matters, maybe it does)
Got the trainstop names right?

I loaded up my SP map and instantly got the Iron Throne 1 achievement even though my iron plate production is just 500/m.

My first guess is that I had 20k iron lying around somewhere so the count of iron in the map instantyl jumped from 0 to 20k+ so the game sees that as production.

Except for the fact that the wiki is still wrong. Some numbers are correct (namely the durations in tick and in seconds) but the percentages are wrong now. (Im talking about the table at https://wiki.factorio.com/index.php?title=Game-day )

I managed to create a solar panel and an accumulator.
